This lady had a large skin cancer removed from the right side of her nose and cheek. With such a large defect the only location to provide this much skin locally is from the forehead, called a paramedian forehead flap. This involves transferring a portion of the forehead skin connected directly to its blood supply to line the nose. For three weeks this leaves the connecting bridge of tissue. This is separated at the three-week mark in the office under local anesthesia. Because of the differences in thickness, frequently a debulking procedure is performed in the office. This helps to contour the reconstructed nose better. In the after photo, she is now three months out from her original reconstruction.
